B. et al. found that asiaticoside reduced mean pulmonary artery pressure and right ventricular hypertrophy by inhibiting the overexpressed TGF-β1/Smad2/3 signaling pathway in the hypoxia-induced pulmonary hypertension rat model. In 2017 they further confirmed that asiaticoside effectively reduced the apoptotic factor (caspase-3), increasing the production of NO by activating the Akt/eNOS pathway. They confirmed that asiaticoside protected pulmonary hypertension by affecting endothelial cell function effect. And asiatic acid has anti-hypertensive and anti-inflammatory effects.
A clinical prospective, placebo-controlled, randomized, dose range trial found that after 4 weeks of treatment with Centella asiatica total triterpenes, the capillary filtration rate, ankle circumference and ankle edema of patients with venous hypertension were improved, and the dose range showed that 180 mg/day was most effective in symptoms improvement.
Atherosclerosis:
Atherosclerosis is a disease in which the inside of arteries narrows due to the buildup of plaque, leading to some serious problems such as heart attack, stroke, or even death. Maintaining arterial integrity and retaining endothelial barrier function and normal contraction of smooth muscle can limit the development of atherosclerotic disease. Clinical studies have shown that after 4 years of intervention in patients with Pycnogenol® 100 mg/day plus Centella asiatica (100 mg/day), the combined treatment group has reduced plaque progression, reduced oxidative stress, and mild transient brain deficiency as compared to the control group. The incidence of angina events in combined treatment group was less than 3%, while the control group it was 6.25%. Arterial Plaques:
Arterial plaques are associated with atherosclerosis, coronary artery disease (CAD), and heart disease in general. When arterial plaques are structurally weak, they are more likely to rupture and cause embolism (artery obstruction/clotting). It has been proposed that a terpenoid extract in this plant may help to stabilize arterial plaque structure by increasing collagen composition. Carotid and Femoral Plaques:
Carotid artery plaques that are echolucent on ultrasound have greater amounts of certain physiological components (e.g., lipids, blood elements) and limited amounts of collagen, making plaque inherently weaker and increasing the risk of embolization. This unstable plaque is associated with a higher clinical risk of stroke and asymptomatic cerebral lesions.
A study on asymptomatic patients (49 men, 38 women; mean age 56) with high-risk, echolucent carotid artery plaques were randomized to receive 60 mg triterpenoid fraction of Centella asiatica or placebo three times daily for one year; patients also took platelet anti-aggregating medication throughout the trial. After 12 months, sonographic evaluation in-dicated a significant decrease in plaque echolucency in the triterpenoid fraction of Centella asiatica group. Incidence of positive MRI images indicating cerebral ischemic lesions was seven percent in the triterpenoid fraction of Centella asiatica group and 17% in the control group.
In a second study testing a similar dose of Centella asiatica, patients with femoral plaques demonstrated a decrease in plaque echolucency after 12 months of therapy, compared to no change in the control group. Degree of stenosis and walking distance did not change in the two groups.
Platelet Aggregation Inhibition:
A 2016 study "In Vitro Antiplatelet Aggregation Activity of Centella asiatica (L.) Urban Ethanolic Extract" published in International Journal of Pharmaceutical and Clinical Research by Agatha Budi Susiana Lestari, Achmad Fudholi, Akhmad Kharis Nugroho, Erna Prawita Setyowati evaluated the anti-platelet aggregation activity of an ethanolic extract of Centella asiatica in male Sprague Dawley rats. In vitro study showed antiplatelet aggregation activity. n-Hexane purification of the CA herbs powder dose not affect antiplatelet aggregation activity significantly.
Thrombosis:

Gotu Kola also seems to increase release of an inflammation mediator called IL-6 (interleukin-6).

In a double-blind study, 94 patients (86 men, 8 women; ages 20-80) with venous insufficiency of the lower extremities for an average of 14 years, were ran-domized to one of three treatment groups: a triterpenoid extract of Centella asiatica asiatica at a daily dose of 60 or 120 mg or placebo for three months. Individuals who took Centella asiatica at either dose demonstrated significant clinical improvements in limb heaviness, edema and global evaluation of efficacy. Venous distension, measured by plethysmography, was significantly better in the active group at 40 mmHg, 50 mmHg and 60 mmHg, compared to deteriorating placebo values.
In a study of 96 patients with venous insufficiency, use of a Triterpenoid Extract of Centella asiatica (TECA) demonstrated significant clinical improvements in limb heaviness, edema, and venous distention. In a study of 40 patients with severe venous hypertension, a trial of the herbal extract showed a decrease in skin flux and rate of ankle swelling, with improvement in edema, restless limbs and change in skin color.
In another study of people with varicose veins, ultrasound tests showed that people who took Gotu Kola had less leakage of fluid.
Chronic Venous Insufficiency (CVI):
Venous insufficiency refers to inadequate blood flow through veins. This condition often results from dysfunction in the veins or one-way vein valves in the legs. Blood pools in the legs unable to return to the heart for pumping and can cause symptoms like burning, aching, pain, stinging, and swelling. One possible cause of venous insufficiency is loss of elasticity in blood vessels.
There is some evidence that Gotu Kola can improve blood circulation in people with Chronic Venous Insufficiency. Another study from the same year, and from some of the same researchers, produced similar results. Venous Hypertensive Angiopathy:
The NMCD rates Gotu Kola as possibly effective for venous insufficiency. This means that preliminary research shows that it works, but more studies are needed to determine efficacy for all people. 40 patients (21 males, 19 females; mean age 48 years) with severe venous hypertension, ankle swelling, and lipodermatosclerosis, were randomized to receive triterpenoid fraction of Centella asiatica 60 mg twice daily or placebo for eight weeks; patients in the study did not wear compression stockings. After trial conclusion, patients taking the herbal extract experienced a significant decrease in skin flux and rate of ankle swelling compared to baseline values. In addition, patients in the active group reported rapid clinical improvement, reflected by a reduction in the analogue scale line score (e.g., symptoms of edema, pain, restless limbs, swelling, and change in skin condition/color) from 9.5 at baseline to 4.5 after eight weeks.
In another study using Laser doppler evaluation, subjects taking 60 mg triterpenoid fraction of Centella asiatica twice daily for six weeks demonstrated a 29% decrease in resting flux, 52% increase in venoar-teriolar response and 66-mL reduction in leg volume 164). Similarly, those utilizing the herbal extract demonstrated 7.2% increase in pO2 and 9.6% reduction in pCO2.

In summary, madecassoside as a triterpene component of Centella asiatica, may have anti-arthritis effect, and the underlying mechanism is reduction in the level of inflammatory factors.

The © Cosmetic Ingredient Review (CIR) Expert Panel reviewed the safety of 9 Centella asiatica-derived ingredients, used primarily as skin conditioning agents in cosmetic products. Panel concluded that CA extract, callus culture, flower/leaf/stem extract, meristem cell culture, meristem cell culture extract, and root extract are safe in present practices of use and concentration in cosmetic products.
Protection From UV Rays:
A 2012 study "Titrated extract of Centella asiatica provides a UVB protective effect by altering microRNA expression profiles in human dermal fibroblasts" published in International Journal of Molecular Medicine by In-Sook An, Sungkwan An, Sang-Mo Kang, Tae-Boo Choe, Sung Nae Lee, Hyun Hee Jang, Seunghee Bae evaluated the photoprotective role of titrated extract of Centella asiatica (TECA) via microRNA expression profiling analysis. Low dose TECA did not exhibit toxicity and showed a protective effect against UVB irradiation. Results suggest TECA may serve as potential natural chemoprotective agent against UVB-mediated damage in NHDFs through changes in expression of specific mRNAs.
Another 2011 study "Triterpene Composition and Bioactivities of Centella asiatica" published in Molecules by Puziah Hashim, Hamidah Sidek, Mohd Helme M. Helan, Aidawati Sabery, Uma Devi Palanisamy and Mohd Ilham evaluated leaves of Centella asiatica for triterpene composition and bioactivity such as collagen enhancement, antioxidant, anti-cellulite and UV protection properties. Study yielded significant amounts of madeccassoside (3.10 ± 4.58 mg/mL) and asiaticoside (1.97 ± 2.65 mg/mL), but low in asiatic and madecassic acid. Highest collagen synthesis was found at 50 mg/ml of extracts. Antioxidant activity was 84% compared to vitamin C (88%) and grape seed extract 83%. Centella asiatica extracts exhibited similar UV protection to OMC at 10% concentration. Results suggest potential application in food and pharmaceutical industries.
Anti Aging:
Extracts of Centella asiatica can combat the signs of aging. It helps the skin to rejuvenate itself and fights against photo-induced aging. Its fluid retention properties trap moisture in the dermal layers and make the skin appear more plump and young. Higher moisture content in the skin cells reduces wrinkles and erases fine lines from the skin. You can work the plant extract into almost any step of your skin-care routine, it’s incorporated into cleansers, toners, serums, and creams. “I like creams and serums because they sit on the skin for a long time for maximum efficacy”, says Nikita Wilson, a cosmetic chemist in New York City. Butler agrees, adding that a cica cream can be used either morning or night, or both. (It’s worth noting that Centella asiatica products are often spiked with a green tint to counteract the redness that comes with an inflamed complexion). It is recommends applying a "cica balm" on top of your retinoid in the p.m. because while retinoids can aggravate skin, the cica will prevent irritation. Things like pollution, cigarette smoke, alcohol, and radiation expose our bodies to things called free radicals, which have been shown to speed up aging and cause illness. But antioxidants, which Gotu Kola contains in spades, are thought to help neutralize the risk. "Gotu Kola's antioxidant properties help us fight signs of aging", says Sanders, who adds that it may even be used to reduce the appearance of cellulite and stretch marks.
A study by Gohil et al. found that Gotu Kola acted on the connective tissues of the vascular wall. In conditions of hypertensive microangiopathy and venous insufficiency, the herb improves microcirculatory parameters by lowering capillary filtration, making Gotu Kola a candidate for reducing the appearance of skin aging.
Rejuvenation:
Regular use of this herb helps in slowing down aging process. Its anti aging properties help to rejuvenate body tissues and boosts body immunity. Lactating mothers can reap the benefits of this herb, as it increases milk secretion and also purifies it. Apart from these benefits the herb Gotu Kola also acts as an antipyretic. There are two specific multi-ingredient cream formulations (trofolastin and verum) containing Centella asiatica that have been observed to reduce the formation of stretch marks during pregnancy. Another topical cream product containing Centella asiatica triterpenes applied twice daily beginning at 10-14 weeks pregnant till the end of pregnancy has been observed to decrease the severity of both new and old stretch marks and the incidence of new stretch marks. These creams also contain other ingredients such as rosehip oil, vitamin E, elastin, and essential fatty acids. The effect of Gotu Kola alone is not known and more research is needed. Preliminary evidence suggests that using a skin cream called Alpha centella two times a day for 6 – 8 weeks might reduce scarring after removal of surgical sutures. This was noted in patients after receiving surgeries to remove excessive scar tissue formations called keloids. More research is needed to determine how gotu kola affects wound healing, scarring, stretch marks, and other dermatological conditions.
Something we find impressive about Centella asiatica is that it is supported by an abundance of clinical trials and studies. You see, we regularly hear about natural ingredients that may or may not be beneficial for stretch marks, but few are actually supported by evidence. Some studies suggested that the terpenoids found in Gotu Kola may help increase collagen production in the body, which may help prevent new stretch marks from forming and reduce the appearance of existing marks. However, it is advisable to do a patch test before using any topical medication.
A recent Indian study found that Centella asiatica was useful in reducing the appearance of stretch marks, especially when applied early during pregnancy. It also showed that Centella is completely safe for use during pregnancy and while breast-feeding, another plus.
In one open clinical trial, 227 patients were divided into two groups and treated with oral Centella asiatica alone or surgical scar revision plus Centella asiatica at doses of 60-150 mg daily for up to 18 months. In the Centella asiatica only group, 116 of 139 patients (82%) experienced relief of symptoms and disappearance of inflammation. In the 88 subjects in the combined surgery and Centella asiatica group, 72% demonstrated improvement. In addition to its oral use, Centella asiatica has been used as a topical cream in a com-prehensive scar management program. Observationally, it was found to improve scar maturity from an average of six months without treatment to 3 months with treatment.
Some studies "Medicinal uses and Toxic effects of Centella asiatica" from 2014 published in Daffodil International University by Jeasminsultana and "Diuretic Activity of Methanolic and Ethanolic Extracts of Centella asiatica Leaves in Rats" from 2011 by Chitrala Roopesh et al. suggest that the use of a special mixture of gotu kola, vitamin E, and a collagen compound in a cream (Trofolastin) in the last six months of pregnancy might reduce stretch marks associated with pregnancy. Another study suggests a mixture of Gotu Kola, vitamin E, essential fatty acids, hyaluronic acid, elastin, and menthol in an ointment forms (Verum®0 might help prevent the same.
In a 2013 review, Gotu Kola claimed to minimize signs of stretch marks. The chemical compound called terpenoids in Gotu Kola increase the production of collagen in the body. This action heals scar and stretch marks and also prevents skin from forming new stretch marks.
Use a topical cream that includes 1% Gotu Kola extract. Apply it to affected areas several times a day for better results.
Skin Diseases:
The Centella asiatica extract and its triterpenoids had certain therapeutic and relieving effects on acne, baldness, vitiligo, atopic dermatitis, and wounds. Acne is one of the most common skin disorders. Studies have pointed out that the activation of vascular endothelial cells and the involvement of inflammation responses are essential for the early stages of the development of acne lesions. Vitiligo is an acquired depigmenting disorder of the skin, and one of the most common skin diseases. Some studies suggested that oxidative stress may be the initial cause of this disease. The main targets of ROS are mitochondria, causing structural and functional changes. Atopic dermatitis, also known as atopic eczema, is a chronic relapsing inflammatory skin condition. This disease occurs due to skin barrier dysfunction, alterations in cell-mediated immune responses, and Immunoglobulin E(IgE)-mediated hypersensitivity. Skin is the largest organ of the human body playing a vital role in maintaining the body’s physiological homeostasis. The appearance of wounds can lead to an imbalance in physiological homeostasis. The stages of wound healing comprise inflammation, proliferation, epithelialization, angiogenesis, remodeling, and scarring.
According to the summary report by 1998 committee for veterinary medicinal products, the transdermal absorption of the active ingredients in Centella asiatica in rats showed that madecassic acid can quickly penetrate the skin barrier, but the dose measured at the drug application point on the skin after 24 hour was only 0.06% concentrated as compared to the original dose. The results of asiatic acid were similar to madecassic acid. The high concentration of asiaticoside administered transdermally did not cause any systemic toxicity, but it could cause excessive keratinization of the skin at the application site. Some reports also suggested allergic dermatitis over using Centella asiatica externally, but none of them reports the exact dose. Centella asiatica and its triterpenoids are made into different formulations to explore the treatment options for skin diseases, and it has been established that they have a potential role in wound healing and skin inflammation.
For wound healing, the present study found that Centella asiatica and its triterpenoids had a direct wound-healing function. Centella asiatica extracted with methanol contains 0.12% asiatic acid, 0.54% madecassic acid, 0.25% asiaticoside and 1.02% madecassoside, and was made into a spray with hydroxypropyl-β-cyclodextrin, Eudragit E100, glycerin, PEG 400, etc., and the spray of triterpenes content are close to 100% compared with Centella asiatica extract, the wound was healed completely without any skin irritation. Compared with ordinary gauze, the electrospun gelatin membranes containing Centella asiatica can promote the wound repair process by affecting the proliferation of fibroblasts and collagen synthesis, and are antibacterial as well. The asiaticoside-rich hydrogel formulation exhibited 40% fast wound healing without any skin irritation as compared to untreated group. Thick epithelial layer and keratin formation can be found, while granulation tissue, fibroblasts and collagen were formed moderately. Cells studies have found that Centella asiatica standard extract (ECa 233) can affect the formation of filopodia and promote wound healing by activating the FAK, Akt and MAPK signaling pathways. In above studies, though the vehicles were different, but animal and cell experiments have found that Centella asiatica and its triterpenoids improved the degree of re-epithelialization, increased the collagen synthesis, reduced the inflammation around wounds and cause no obvious skin irritation.
For the treatment of atopic dermatitis, Centella asiatica significantly reduced the inflammation response (TNF-α ↓, IL-1β ↓, IL-8 ↓, IL-4 ↓, and IL-13 ↓), and also the local immune response (IgE ↓). Whether titrated extract of Centella asiatica or ethanol extract of Centella asiatica, both seems to inhibit hyperkeratosis, mast cell and inflammatory cell infiltration. Both of them can inhibit the expression of iNOS and COX-2 and NF -κB activity, it confirms that Centella asiatica extract may be a promising therapeutic for the treatment of atopic dermatitis. The effect of madecassoside in the treatment of dermatitis is reflected in reducing the pro-inflammatory cytokines (IL-1β, TLR2), moreover, it can promote the secretion of AQP3, LOR, IVL in HaCaT keratinocytes and the secretion of HA in human skin fibroblasts, thus can significantly enhance skin hydration.
Madecassoside, a specific component of Centella asiatica, had a certain improvement effect on vitiligo, and the possible mechanism of action was to reduce the oxidative stress response and weaken the damage to mitochondria by oxidative stress [matrix metalloproteinase (MMP) ↑ and [Ca2+]i ↓]. In addition, it was found that the LC3-II/LC3-I ratio of melanocytes treated with madecassoside increased significantly, suggesting that it enhances the autophagy activation of the cells, thereby protecting skin cells from physiological and pathological aging damage. Lastly, Centella asiatica also demonstrated a positive activation effect on dermal papilla, improved the viability of dermal papilla cells and increased the expression of characteristic genes related to hair growth in the cells, thus providing good application prospects for baldness.
Although Centella asiatica and its triterpenoids have low transdermal absorption rate, current animal experiments and cell experiments have found that they can effectively promote wound healing, reduce skin inflammatory diseases, and seem to have a certain effect on vitiligo and baldness. The mechanism of action of Centella asiatica and its ingredients in the treatment of skin diseases is mainly anti-inflammation, anti-oxidation, and weakening of the damage to mitochondria by oxidative stress, which was consistent with the pathogenesis of these diseases.

Psoriasis:

A 2011 study "Diuretic Activity of Methanolic and Ethanolic Extracts of Centella asiatica Leaves in Rats" published by Chitrala Roopesh et al. suggest use on gotu kola on the skin might reduce the symptoms of psoriasis.
It is the best remedy for skin diseases like eczema and psoriasis. Boil Centella leaves with cow ghee and make it cool and apply on the skin.
Scars:
Oral and topical use have reported benefits on wound and scar management with relief of symptoms, disappearance of inflammation and hastening of scar maturity.
Wounds, Burns and Injuries:
As kids, we are always falling and covered in bruises and wounds. What our elders would do is take a bunch of Gotu Kola leaves, moisten the herb with saliva chew it for a while. With chewing it becomes dark green in colour. This is then taken out and applied on the wound with a bandage wrapped around it. The bandage is left for a day. Depending on the intensity of the wound, the dressing is changed. Gotu Kola has long been used traditionally in many civilizations to heal wounds, both internally and topically. Once famous for its use in treating leprosy in India, in Ayurveda, Gotu Kola is used today by herbalists to treat burns, minimize scarring, heal wounds, and promote tissue repair after injury or surgery. It appears to promote wound healing through its anti-inflammatory, antioxidant, and antibacterial qualities, in addition to stimulating keratinization (an integral process of nail and hair growth) and epidermal repair (the epidermis is the outermost layer of the skin). The main component asiaticoside helps in increasing type 1 collagen formation and angiogenesis thus heals wound rapidly. These compounds also inhibit inflammatory and improve capillary permeability. Its use is recommended in open sores and chronic wounds. A set of lab studies and medical records showed that the Gotu Kola extract could regenerate connective tissues to generate new skin and stimulate biological processes in cell division to heal wounds rapidly.
Pharmacological experiments have proven that the increased antioxidant activity of Gotu Kola indicates the wound healing properties of Gotu Kola extract. Gotu Kola provides the same healing property when consumed orally. When taken in its natural form or powdered form, the herb aid in stimulating blood flow to the cells and protects your immune system against infections, thus, speeding up the healing process in your body.
According to Gaia, the herb supports the structural integrity of skin and connective tissues. It is also purported to maintain normal skin keratinization, which is important for formation of nail and hair tissue.
A clinical study was done on wounds like clean cuts by sharp objects, blunt force trauma injuries, and infected rats. The wounds were dressed with Gotu Kola or Centella asiatica herb dressing and were seen to have improved wound healing qualities.
In in vitro and animal research, Centella asiatica has been shown to increase type 1 collagen synthesis, relative to type 2 collagen synthesis. Type 2 collagen is linked to hypertrophic scarring; meaning excessive scar tissue growth. By increasing type 1 collagen, scarring may be reduced. Both animal and preliminary human research has shown Gotu Kola to speed up wound healing. These effects are attributed to collagen preserving/increasing effects. Gotu Kola also seems to tighten the skin, helping to smooth out wrinkles and increase firmness. Asiaticoside constituents in Centella asiatica might decrease wound healing times by upregulating the production of collagen and of long, unbranched polysaccharides called glycosaminoglycans. This increased production of collagen is thought to have beneficial effects elsewhere in the body as well.
The Memorial Sloan Kettering Cancer Center also highlight the lack of trials in humans and call for further testing before the effectiveness of Gotu Kola as a wound treatment can be confirmed. Centella asiatica extract is able to penetrate the skin well enough to be utilized effectively and has been known to treat wounds and burns for centuries. This has been shown in both in vitro and in vivo studies which back its effectiveness in wound healing. The way it does that is by promoting fibroblasts proliferation and collagen synthesis, as well as inhibiting the inflammatory phase of scars. It has also been observed that the triterpenoid components stimulate glycosaminoglycan, especially hyaluronic acid production.
An animal trial reported that different formulation of Gotu Kola topically applied on the open wound thrice daily for 24 days treatment period showed increase skin tissue growth with collagen synthesis at the site of the wound with increase tensile strength.
One in vitro study isolated a major constituent in Gotu Kola, asiaticoside, and demonstrated the proliferation of fibroblasts, which are specialized cells responsible for producing and maintaining the structure of connective tissue. Fibroblasts are integral to wound healing.

Urban herb" published by Bogdan Kędzia, Teresa Bobkiewicz-Kozłowska, Mirosława Furmanowa, Przemysław Mikołajczak, Elżbieta Hołderna-Kędzia, Irena Okulicz-Kozaryn, Joanna Wójcik, Joanna Guzewska, Waldemar Buchwald, Alina Mścisz, Przemysław M. Mrozikiewicz evaluated about wound healing activities of different extracts of Centella asiatica in incision and burn wound models.
In Asia, people often call Centella asiatica "tiger grass", believing that injured tigers roll in the herb because of its healing properties. Several reports offer evidence for these allegedly magical effects. Gotu Kola has another benefit in this healing arena: Its adaptogen-like qualities help with the emotional and physiological stresses of physical trauma. Calendula flowers (Calendula officinalis) also promote tissue repair and support the lymphatic system in fighting infection, so I frequently pair them with Gotu Kola. Along with Gotu Kola’s wound healing properties, it’s also applied topically to mollify a variety of skin conditions, including insect bites, seborrheic dermatitis, cold sores, eczema, psoriasis, and dry, irritated skin. I like to infuse the dry herb into sesame or coconut oil, which can be rubbed into the scalp to calm the mind, deepen sleep, and promote hair growth.
The University of Maryland Medical Center states that Gotu Kola has been used traditionally to improve wound healing and enhance mental clarity, as well as for various skin conditions including leprosy, psoriasis, eczema, and acne.

The authors of an April 2018 study published in Minerva Chirurgica evaluated Centella asiatica as a treatment to decrease surgical scarring. Patients receiving oral doses of the herb healed better and experienced less scarring.
A report published in the March 2017 edition of The Journal Medicine showed that burn patients directly applying Centella asiatica had more complete healing. Such application also decreased the risk of infection.
A 2016 study "Evaluation of the topical spray containing Centella asiatica extract and its efficacy on excision wounds in rats" published in Acta Pharmaceutica by Somchai Sawatdee, Kanuengnit Choochuay, Wirot Chanthorn, Teerapol Srichana evaluated the wound healing effect of a topical spray formulation on Centella asiatica. The extract was complexed with hydroxypropyl-ß-cyclodextrin (HP-ß-CD) and formulated with Eudragit E100, glycerol, PEG 400, copovidone, ethanol, and purified water. Skin irritation study showed the formulation was non-irritating in the rat model. Results showed complete healing after 14 days in the in vivo excision wound model.
A 2015 study on rats found that wound dressing containing Gotu Kola was effective in healing multiple types of wounds, including clean cuts, irregular tears and infected tissue, and blunt force trauma caused tears. However, most of the studies supporting this health benefit of Gotu Kola were conducted on animals or used artificial wounds created in labs. This means there is no good scientific evidence to support its use in humans.
A 2013 study "Centella asiatica in cosmetology" published in Postep Derm Alergol by Wiesława Bylka, Paulina Znajdek-Awiżeń, Elżbieta Studzińska-Sroka, Małgorzata Brzezińska about mechanisms where Centella asiatica has been found effective in the treatment of small wounds, hypertrophic wounds, burns, psoriasis, and scleroderma. This review paper describes the mechanisms involved i.e., promotion of fibroblast proliferation, increase of collagen synthesis and intracellular fibronectin content, and improved of tensile strength of newly formed skin, along with inhibition of inflammatory phase of hypertrophic scars and keloids. Studies have suggested potential use in the treatment of photoaging skin, cellulite, and striae.
Another 2012 experimental animal study "Wound healing activities of different extracts of Centella asiatica in incision and burn wound models" published in BMC Complementary and Alternative Medicine by Juraiporn Somboonwong, Mattana Kankaisre, Boonyong Tantisira and Mayuree H Tantisira evaluated the wound healing activities of various extracts of Centella asiatica in incision and partial-thickness burn wound models in rats. Results showed all extracts facilitated wound healing process in both models. The asiatic acid in the ethyl acetate extract seemed the most active component for wound healing. An aqueous-ethanol extract showed bacteriostatic activity to all gram-negative and gram-positive bacteria isolated from wounds of different etiology. Applied topically as 1% cream the extract showed tendency to reduce carrageenan-induced edema.
According to a 2010 review of studies, some research supports the use of Gotu Kola to help wounds and burns heal. The authors reported that many studies had found that Gotu Kola sped up wound healing. However, most of these studies involved animals or artificial wounds created in labs. This means that the herb may not have the same effects in humans. Somayaji, Msc, PhD was showed the leaf extract of Centella asiatica significantly promoted wound healing and was able to overcome the wound-healing suppression of dexamethasone.
You can find Gotu Kola in many creams for wound healing. Ask your health care provider if one is right for you. Apply an ointment that contains 1% Gotu kola extract on wounds several times a day. In addition to its internal use in healing injuries, Gotu Kola is used topically, in the form of poultices, compresses, soaks, and infused oils (don’t use infused oils on fresh burns). But the dose used in this study was very high, so it is impossible to say how Gotu Kola might be used to treat anxiety.
In vitro studies have shown that Centella asiatica may bind to both GABA (Gamma Aminobutyric Acid) and CCK (Cholecystokinin) receptors. Stimulating GABA receptors may be responsible for the anti-anxiety effect. It has been hypothesized that activation of GABA receptors may also promote analgesic (pain-relieving), anticonvulsant, and sedative effects, however this hypothesis has not been tested in any research trials yet.

In 2016, an animal study was performed to witness the anti-anxiety ability of Gotu Kola in male mice that had undergone sleep deprivation for three days (72 hours). Sleep deprivation is one of the primary reasons for anxiety, neuroinflammation, and oxidative damage. The subjects were consumed Gotu Kola continuously for five days before they went to sleep deprivation. The result shows a significant reduction in abnormal anxiety behaviors. The mice showed improvement in oxidative damage and their locomotor activity.
In 2013, researchers reviewed various anti-anxiety medicines and herbs like Gotu Kola and concluded the herb has active anti-anxiety properties.
A 2000 study supports this ancient claim. After assessing baseline measurements of Acoustic Startle Response (ASR), mood self-rating scale, heart rate, and blood pressure, 40 healthy subjects (21 males, 19 females; ages 18-45 years) were randomized to receive 12 g non-standardized Centella asiatica dissolved in 300 mL grape juice or placebo. Evaluations were re-corded at 30, 60, 90, and 120 minutes after beginning therapy. Centella asiatica significantly decreased acoustic startle response amplitude compared to placebo at 30 and 60 minutes; heart rate, blood pressure, and mood did not change.
A 2000 double-blind, placebo-controlled study on the effects of Gotu Kola (Centella asiatica) on Acoustic Startle Response in Healthy Subject on rats published in Journal of Clinical Psychopharmacology by Bradwejn J, Zhou Y, Koszycki D, Shlik J. has shown decrease in locomotor activity, enhanced maze performance and attenuated started response. This double-blind placebo- controlled study evaluated the anxiolytic activity of Gotu Kola in healthy subjects. Results showed Gotu Kola has anxiolytic activity in humans as shown by the Acoustic Startle Response (ASR). The study compared with placebo, Gotu Kola significantly attenuated acute acoustic response. Gotu Kola has anxiolytic activity in humans. It remains to be seen whether it has efficacy for use in anxiety syndromes.
In another 2000, The Journal of Clinical Psychopharmacology published research claiming that those who take Pennywort showed a significant reduction in their panic disorders. That means that Pennywort contains triterpenoids that help reduce anxiety and improve mental health.
Take 50 mg of Gotu Kola extract two times a day as a single dosage for two weeks (14 days). In the case of extreme anxiety, it is advised to take up to 2000 mg per day.

Another 2016 study "To evaluate and compare antidepressant activity of Centella asiatica in mice by using forced swimming test" published in IJBCP: International Journal of Basic & Clinical Pharmacology by Padmavathi Golla, Hemapriya Tirupathi evaluated the antidepressant-like effect of C. asiatica using forced swimming test (FST). Doses of 10 mg and 20 mg/kg significantly decreased immobility time, suggesting antidepressant activity. At 20 mg/kbw, the antidepressant activity was almost similar to standard drug imipramine (10 mg/kg).

A 2017 systematic review and meta-analysis "Effects of Centella asiatica (L.) Urb. on cognitive function and mood related outcomes" published in Scientific Reports, Vol. 7 by Panupong Puttarak, Piyameth Dilokthornsakul, Surasak Saokaew, Teerapon Dhippayom et al. where Centella asiatica has been used as a brain tonic for mental disorders and enhancement of memory. This systematic review includes five randomized controlled trials conducted to determine the effects of Centella asiatica on cognitive function and its related properties. Meta-analysis indicated that there are no significant differences in all cognitive function domains of Centella asiatica when compared to placebo. However, it could improve mood by improving alertness and relieving anger. There are limitations on dose regimens, plant preparation, standardization and product variation. Well-designed trials are suggested. The review described the herb's alerting properties. In human trials, daily use of Centella asiatica increased feelings of alertness and calmness. It also had a positive effect on anger management. These calming effects appeared within a few minutes of taking the herb. The authors placed great emphasis on the safety of Centella asiatica. Multiple studies showed that the participants taking the herb reported no side effects. Yet, physicians caution about the remote possibility of liver damage.

A 2010 study "Anti-ulcer activity of Centella asiatica leaf extract against ethanol-induced gastric mucosal injury in rats" published in Journal of Medicinal Plants Research Vol. 4 by M. A. Abdulla, F. H. AL-Bayaty, L. T. Younis and M. I. Abu Hassan evaluated the anti-ulcerogenic activity of an ethanol extract of Centella asiatica against ethanol induced gastric mucosal injury in rats. Pretreatment of leaf extract showed significant gastric mucosal protection, reduction or absence of edema and leukocytes infiltration of submucosal layer.
Fresh juice extract of Gotu Kola has shown protection against aspirin- and ethanol-induced gastric ulcers with increased gastric mucin secretion and mucosal cell glycoprotein production.

An animal study in 2017 demonstrated that Gotu Kola is capable of reducing the toxin side effects of antibiotic isoniazid. The rats were ingested 100 mg Gotu Kola for a month, and after that, an antibiotic was given to them. After a study period, the rats showed fewer toxicity levels. The rats with liver and kidney toxicity experienced reduced or normalized toxin levels after intaking Gotu Kola.

Chronic and recurrent liver injuries are often accompanied by inflammatory reactions and often develop into liver fibrosis. Therefore, treating chronic and uncontrolled inflammation is a strategy to prevent liver injury and fibrosis. The pathological mechanism of gastric mucosal injury is complex, and nonsteroidal anti-inflammatory drugs are relatively common causes. Prostaglandin biosynthesis is one of the basic components that maintain the integrity of gastric mucosa, and cyclooxygenase is essential in the process of prostaglandin synthesis. The malondialdehyde level can reflect the ROS level. Therefore, an oxidative stress response is also crucial in the process of gastric mucosal injury. The present study found that the Centella asiatica extract effectively ameliorated the drug-induced liver toxicity, improved gastric mucosal injury, and reduced H. pylori infection. The mechanisms involved were as follows: the reduction of related inflammation factors (IL−1β ↓, IL−2 ↓, IL−6 ↓, IL−10 ↓, IL−12 ↓, and TNF−α ↓) and the increase in the level of antioxidant stress factors (SOD ↑, CAT ↑, and GPx ↑). A 2007 study "Evaluating the effects of Centella asiatica on spermatogenesis in rats" published in Journal of Reproduction & Fertility by Mahnaz Heldari, Amir Hasan Jamshedi, Shahin akhondzadeh, Maerefat Ghaffari et al. evaluated the effects of C. asiatica extract on spermatogenesis and testicular tissue of rats. Results showed a calculated plant LD50 of 500 mg/kg. Results showed significant increase in body and testis weight in rats. Histological changes during spermatogonal evolution were degeneration of spermatozoa and interstitial congestion in some tubules. Sperm analysis showed decrease in number of spermatozoa (p<0.01), motile sperms (p<0.001) and epididymal sperm storage (p<0.001) with no changes in sperm morphology. Results suggest potential as temporary contraceptive agent in animals.

A 2002 placebo-controlled double-blind clinical trial "Evaluation of the Efficacy of Mentat in Children with Learning Disability" published in Neurosciences Today by Upadhyay, S.K., Bhatia, B.D., and Kala Suhas Kulkarni, M.D. about improvement of learning disability: Mentat is a polyherbal formulation containing Centella asiatica, together with B. monnieri, W. somnifera, N. jatamansi, A. calamus, T. cordifolia, E. officinalis, T. arjuna among others. Centella asiatica contains brahmic acid, isobrahmic acid, brahmoside and brahminoside. A double-blind placebo controlled trial evaluated the efficacy of Mentat in children with learning disability. Mandukaparni (Centella asiatica) is valued in Indian systems of medicine for improving memory and treatment of nervine disorders. This study reports on a plant material sold in the name of Vallarai that was not Centella asiatica and identified as Merremia emarginata (Kupit-kupit), a trailing plant that resembles Centella asiatica. The study stresses the importance of standardization of herbal drugs at raw drug levels by use of taxonomic and other validation methods.
A 2010 study "Enhancement of memory in rats with Centella asiatica" published in Biomedical Research Volume 21, Issue 4 by Silviya Rajakumari Jared evaluated the effect of C. asiatica on learning ability and memory in male Wistar rats measuring two important parameters using Operant conditioning technique, viz., latency period and total number of bar pressings. Results conclude that Centella asiatica facilitated retention of a learned task for a longer period suggesting good retention of memory but did not accelerate the learning process as expected.

Gray, Armando Alcazar Magana, Parnian Lak, Kirsten M. Wright, Joseph Quinn, Jan F. Stevens, Claudia S. Maie, Amala Soumyana describes the phytochemistry and neurological effects of Centella asiatica. Many studies in rodent models and some human studies support Centella asiatica as a cognitive enhancer, anxiolytic, and anticonvulsant. A 2013 study "Neuritogenic effect of standardized extract of Centella asiatica ECa233 on human neuroblastoma cells" published in BMC Complementary and Alternative Medicine by Oraphan Wanakhachornkrai, Varisa Pongrakhananon, Preedakorn Chunhacha, Aree Wanasuntronwong, Anusara Vattanajun, Boonyong Tantisira, Pithi Chanvorachote and Mayuree H Tantisira, showed neurite outgrowth promoting activity of Eca 233. Besides the in vivo neuroprotective effect of ECa 233, study supports further development of ECa 233 for clinical use in neuronal injury or neurodegenerative diseases.
"Effect of Centella asiatica leaf powder on oxidative markers in brain regions of prepubertal mice in vivo and its in vitro efficacy to ameliorate 3-NPA-induced oxidative stress in mitochondria" published in International Journal of Phytotherapy and Phytopharmacology, 2008, Vol. 15, Issue 11 by Shinomol, George K. evaluated the propensity of dietary intake of Centella asiatica leaf powder to modulate oxidative markers in mouse brain regions and the efficacy to abrogate 3-NPA induced oxidative stress in mitochondria. Results showed Centella asiatica has the propensity to modulate both endogenous and neurotoxicant induced oxidative impairments in the brain and may have potential as neuroprotective adjuvant to abrogate oxidative stress in vivo.

Oxidative stress is mainly caused by lipid peroxidation and has been considered as the main indicator of the pathogenesis and development of type 2 diabetes. Oxidative stress causes microvascular and macrovascular complications. In addition, the inflammation response may cause the occurrence of type 2 diabetes by inducing insulin resistance. The inflammation response is exacerbated in the presence of hyperglycemia and can, in turn, worsen hyperglycemia. Hence, targeting the inflammation pathway may be a potential strategy to prevent and control diabetes.

Chauhan, I.P. Pandey and Vinod Kumar Dhatwalia evaluated ethanol and methanol extracts of leaves for anti-diabetic activity in normal and alloxan induced diabetic rats. In addition, VAR scores (decrease of skin blood flow on standing) increased significantly from 6.4 percent to 23.9% at three months and 25.9% at 6 months. During the investigation period, pO2 increased while pCO2 values decreased significantly in the Centella asiatica group. Fasting blood sugar and hemoglobin A1C values did not change.

Colon cancer and primary liver cancer are common types of cancers in the digestive system. The expression of E-cadherin and vimentin is considered of high reference value in the prognosis of colon cancer. The mitochondrial morphology and the cytosolic calcium level [Ca2+] are indicators of the pathological development of hepatocellular carcinoma. Asiatic acid can also affect the expression of epithelial-mesenchymal transition marker proteins in colon cancer cells (E cadherin↑, vimentin ↓, N-cadherin↓), achieved this anti-cancer potential by regulating PI3K/Akt/mTOR/p70S6K signaling pathway. "Anti-carcinogenic activity of Centella asiatica and Elytropappus rhinocerotis on a human colon cancer cell line" published in Theses and dissertations (Applied Sciences) - DUT Durban Library by Dwarka, Depika, an Anti-Carcinogenic on Human Colon Cancer Cell Line Study investigated the anticancer potential of Centella asiatica and Elytropappus rhinocerotis. Results showed the plants induces apoptosis in CaCO-2 cells, an important step in elucidating the underlying molecular mechanism for anti-tumor activity.
Ovarian Cancer and Breast Cancer:
Ovarian cancer and breast cancer are the top two cancers in women. Surgery, radiotherapy, chemotherapy, and other treatments are often associated with various complications. Therefore, exploring new therapeutic agents is particularly important. The potential therapeutic value of asiatic acid for ovarian cancer is mainly reflected in that it can promote the apoptosis of ovarian cancer cells and inhibit the growth of ovarian cancer cells by affecting the cell cycle progression.
In test tube studies, asiatic acid in Gotu Kola showed its ability to induce natural cell death (apoptosis) in specific cancer types like breast cancer. In addition, studies found that asiatic acid improved the developmental ability of early embryos in pigs; the main underlying mechanism was amelioration of oxidative stress and downregulation of the expression of apoptosis-related genes. The potential therapeutic value of Centella asiatica for breast cancer mainly reflected in the promotion of apoptosis of breast cancer cells.

A review found that pretreatment with asiatic acid can inhibit bleomycin-induced lung injury and fibrosis in mice. It can down-regulate the expression of pro-inflammatory factors, inhibit inflammatory cells infiltration and expression of transforming growth factor-β1. In a mouse model, lung inflammation was induced by exposure to cigarette smoke, oral administration of asiatic acid reduced the excessive production of mucus in lung tissues, inhibited the release of pro-inflammatory factors, and induce the expression of HO-1, which may become a potential drug for the treatment of COPD by regulating key progressions (Lee et al., 2016; Dong et al., 2017). The common possible mechanism was that asiatic acid could reduce the level of related inflammation factors (IL-6 ↓, TNF-a ↓). In addition, asiatic acid can inhibit collagen deposition in lung fibrosis diseases. Cell and animal experiments found that asiatic acid could reduce tumor volume, tumor migration, and differentiation. Furthermore, it also has the ability to promote tumor cell apoptosis (caspase-9 ↑, caspase-3 ↑). Therefore, asiatic acid may be a potential therapeutic drug for lung cancer. As for asiaticoside, the review found that it can reduce the inflammatory infiltration caused by lipopolysaccharide in a dose-dependent manner, and inhibit the inflammatory response in lung tissue by inhibiting the NF-κB signaling pathway (TNF-α ↓, IL-6 ↓, p-IκB ↓, p-p65 ↓), which can be an effective preventive agent for acute lung injury.
A 2013 study of an aqueous extract of Centella asiatica "Anti-Proliferative Activities of Centella Asiatica Extracts on Human Respiratory Epithelial Cells in vitro" published in Int. J. Morphol. by P. M. Mutua; M. M. Gicheru; A. N. Makanya and S. G. Kiama demonstrated a dose-dependent inhibitory effect towards the proliferation activities of human respiratory epithelial cells. Results suggest the possibility of using Centella asiatica extract as an anti-polyps therapy.
Therefore, the present studies showed that the effective components of Centella asiatica on respiratory diseases were asiatic acid and asiaticoside, and the major mechanism was anti-inflammation; this was also consistent with the pathological mechanism of the aforementioned diseases. Moreover, it is worth paying close attention to the potential therapeutic effect of Centella asiatica and asiatic acid on lung cancer. Also, the mechanisms of promoting apoptosis and inhibiting differentiation of tumor cells are worthy of further exploration.
Pulmonary Fibrosis:
Pulmonary fibrosis can be induced by a variety of injuries to the lung. It is characterized by fibroblast/myofibroblast activation and excessive extracellular matrix accumulation, leading to a progressive organ dysfunction mainly including varying degrees of inflammation and fibrosis. By reducing collagen accumulation, pulmonary fibrosis can be effectively improved through TGF-β1 and NLRP3 pathways and decreasing the levels of inflammation factors.
Chronic Obstructive Pulmonary Disease (COPD):
Chronic Obstructive Pulmonary Disease is a frequently progressive inflammatory disease of the respiratory tract, alveoli, and microvasculature. The pathological mechanism of COPD is that airway epithelial cell damage triggers nonspecific inflammatory responses by releasing endogenous intracellular molecules or molecular patterns associated with danger. Impaired immune regulation may play a major role in COPD. For the treatment of COPD, it is generally recommended to use appropriate long-acting maintenance bronchodilators and inhaled corticosteroids; pulmonary rehabilitation can also relieve symptoms. However, corticosteroid treatment has certain side effects. Lung cancer is the most common cause of cancer-related mortality in the world. Different treatment methods for lung cancer are generally selected according to the stages, such as surgery, radiation therapy, molecular targeted therapy, and immunotherapy.
Acute Lung Injury (ALI):
Acute lung injury is a systemic inflammation of the lungs manifested as hypoxia, edema, and pulmonary infiltrates present in the chest cavity. Results also showed Centella asiatica extract and powder may ameliorate H2O2-induced oxidative stress by increasing HDL concentration, decreasing TG, LDL, lipid peroxidation. Effects are attributed to antioxidant components and polyphenols substances present.
A 2007 study "Protective effect of Centella asiatica extract and powder on oxidative stress in rats" published in Food Chemistry Vol 100, Issue 2 showed Centella asiatica extract and power may ameliorate H202-induced oxidative stress by decreasing lipid peroxidation.

Gotu Kola protects the brain from the harmful effects of free radicals. Urban in single cell gel electrophoresis assay" published in Spatula DD. by Tejpal Gill, Manish Kumar, Subodh Kumar, Satwinderjeet Kaur. evaluated an aqueous methanolic extract and various fractions against hydrogen peroxide (H2O2) induced DNA damage. Results showed potent dose-dependent genoprotective activity. Centella asiatica showed to be a strong modulator of genotoxicity caused by oxidative mutagen and presents a potential for use in chemopreventive trials.
Another 2009 study "Effect of Centella asiatica L. extract against ethinylestradiol induced genotoxic damage on cultured human lymphocytes" published in Biomedical Research: An international Journal of Medical Sciences by Yasir Hasan Siddique, Tanveer Beg, Mohammad Afzal evaluated the effect of Centella asiatica extract against genotoxic doses of ethinylestradiol on human lymphocytes culture. Results suggest Centella asiatica extract reduced genotoxic damage during ethinyl- estradiol therapy in patients, thereby reducing the chances of cancer development in humans.
Kills Bacterial Infections:
Extracts obtained from Centella asiatica have also been found to have a bacteriolytic effect.

A 2009 in Bovine Mastitis study "Antibacterial Activity of Crude Extracts of Centella asiatica against Staphylococcus aureus in Bovine Mastitis" published in Kasetsart Veterinarians vol. 19 No. 3 by Duangkamol Taemchuay, Theera Rukkwamsuk, Thavajchai Sakpuaram and Nongluck Ruangwises, evaluated crude ethanol or water extracts for in vitro antibacterial activity against 30 isolates of Staphylococcus aureus from milk samples of dairy cows. Results showed antibacterial activity with the ethanol extracts showing more potential antibacterial activity against Staphylococcus aureus than the water extracts. Results suggest testing in an in vivo study.
"A study on antibacterial activity of crude extracts of asiatic pennywort and water pennywort against Staphylococcus aureus" published in KMITL Sci. J. Vol 8, No 2, by Duangkamol Taemchuay et al. about crude water extract of Asiatic Pennywort, particularly extracted with water, showed promising antibacterial effect against Staphylococcus aureus.
An in vitro study of the effect of Centella asiatica (Indian pennywort) on enteric pathogens published in Indian J Pharmacol Vol 36, Issue 1 in 2004 on the effect of Centella asiatica on enteric pathogens. The alcohol extract was bactericidal against V cholera, Shigella spp, and Staph aureus and suggests further studies in its potential as an antidiarrheal drug.
Antimicrobial:
A 2009 study "Antimicrobial and Antifungal Activity of Centella asiatica (L.)Urban, Umbeliferae" published in Research J. Pharm. and Tech.2 by NS Jagtap, SS Khadabadi, DS Ghorpade, NB Banarase, SS Naphade evaluated the antimicrobial activity of various extracts against selected strains, viz., P. vulgaris, S. aureus, E. coli, B. subtilis, A. flavus, A. niger, and C. albicans. Results showed the ethanolic extract to have higher antimicrobial activity than petroleum ether and water extract.
An in vitro study of the effect of Centella asiatica (Indian pennywort) on enteric pathogens published in Indian J Pharmacol., February 2004, Vol 36, Issue 1 by B. Mamtha, K. Kavitha, K. K. Srinivasan, P. G. Shivananda demonstrated the antimicrobial activity of Centella asiatica against enterophathogens. Broad spectrum activity of the herb was observed against a range of enteric pathogens. Against V cholera, Shigella species and S aureus, the alcoholic extract was bactericidal within 2 hr.
