Overall reduction in stress symptoms by 72.3% versus a reduction of 2.3% for the placebo group. Similarly, a reduction of symptoms by 71.6% on the Depression-anxiety stress scale was noted for the group taking ashwagandha versus the placebo group.
The outcomes measured in the study included levels of the stress hormone cortisol, side effects, as well as participant perceptions of stress levels and general health as measured by standardized questionnaires. The group taking Ashwagandha experienced a 44% reduction in self-ratings of stress levels according to the Perceived Stress Scale versus the placebo group, which experienced only a 5% reduction in stress levels. Serum Cortisol levels, useful to assess the effect of the treatment on cortisol, the stress hormone in your body, level, which dropped by 27.9% over 60 days.
Limited research from 2013 also suggests that Ashwagandha may help improve cognitive impairment in people with bipolar disorder. However, more research is needed.
"A prospective, randomized double-blind, placebo-controlled study of safety and efficacy of a high-concentration full-spectrum extract of ashwagandha root in reducing stress and anxiety in adults" published in Indian Journal of Psychological Medicine, Volume 34, issue 3, July 2012 by Chandrasekhar, K. et al. concluded taht the findings of this study suggest that a high-concentration full-spectrum Ashwagandha root extract safely and effectively improves an individual's resistance towards stress and thereby improves self-assessed quality of life.
A 2009 study "Nutrients and Botanicals for Treatment of Stress: Adrenal Fatigue, Neurotransmitter Imbalance, Anxiety, and Restless Sleep" published in Alternative Medicine Review, Volume 14, Issue 2 by Head K. and Kelly, G. has shown anti-stress and anabolic activity of Ashwaganda.
Another 2009 randomized controlled trial "Naturopathic Care for Anxiety" published in PLOS One by Cooley, K. et al. showed when anxiety levels were measured after the 12-week period, the group who received Ashwagandha had anxiety scores that decreased by 55% and the psychotherapy group’s scores decreased by 30.5%. Showing that Ashwagandha can even outperform psychotherapy. 75 participants (93%) were followed for 8 or more weeks on the trial. Final BAI scores decreased by 56.5% (p<0.0001) in the NC group. Significant differences between groups were also observed in mental health, concentration, fatigue, social functioning, vitality, and overall quality of life with the NC group exhibiting greater clinical benefit. No serious adverse reactions were observed in either group.

Another 2000 A double-blind, placebo-controlled evaluation of the anxiolytic efficacy ff an ethanolic extract of Withania somnifera published in Indian Journal of Psychiatry, Volume 42, Issue 3 by Andrade, C. et al. it is concluded that this ethanolic extract of Withania somnifera has useful anxiolytic potential. For over 6 weeks, 88.2% of the people who took Ashwagandha reported a reduction in anxiety.
The limited research available suggests that Ashwagandha may help reduce symptoms of depression and benefit people with some mental health conditions. However, more research is needed.
